Thank you for your inquiry regarding a support staff position in the Pleasant Valley School District. To consider you for possible employment, the documents listed below must be received in our Personnel Office:
Completed Pleasant Valley School District General Employment Application.
Copy of current Act 34 State Police Criminal Record Check (SP-4-164).
Copy of current Act 151 Pennsylvania Child Abuse History Clearance (CY 113).
Copy of current Act 114 Federal Bureau of Investigation (FBI) Federal Criminal History record. The program for the Commonwealth. The registration process is available online 24 hours/day, seven days per week at www.pa.cogentid.com. Telephone registration is available at 1-888-439-2486 Monday through Friday, 8:00 am am to 6:00 pm EST.
Copy of transcripts from all colleges universities attended (when applicable).
All active applications (those which include materials in 1-5) will be kept on file for one year following the date receipts in writing, along with an updated resume and any other amended information.
Invitation for personal interviews will be extended to those candidates who appear to best meet our needs. If you are contacted for an interview, you will need to bring with you:
Original Act 34-PA Criminal Record Check
Original Act 151-PA Child Abuse History Clearance
Original Act 114-FBICriminal History Record
Your previously submitted copies will be initialed and all original documents returned to you.
Upon hire by the district, you are responsible for submitting to the Personnel Office these additional documents:
U.S. Department of Homeland Security/Employment Eligibility Verification (Form I-9) and originals of identification to establish employment identify.
School Personnel health Records (H511.340), including current TB test
Official transcripts from all college/universities attended (when applicable)
If you already have a physical examination and tuberculosis test within three (3) months, a copy of that School Personnel Health Record will satisfy this requirement. All required forms can be obtained in the Personnel Office.
Failure to present, by hire date, a PA Criminal Record Check, PA Child Abuse History Clearance, FBI Criminal History Record, identification documents to establish employment identity and eligibility (Form I-9), transcripts from all colleges/universities, a School Health Personnel Health Record including current TB test, may result in a provisional hiring status. Failure to submit these original documents within the legal and district established time lines would result in termination of employment.
Individuals interested in being placed on our substitute list should indicate so in writing, either on the application or by letter in addition to submitting all of the aforementioned documents.
