Form I-9 must be complete to document verification of identity and employement authorization of each new employee. The acceptable documents listed on page three (3) must also accompany this form.

All positions require the submittal of the original results of a physical examination, including a current TB test on a School Personnel Health Record Form. If you have had a physical examination within one year and turberculosis test within 90 days, a copy of that School Personnel Health Record will satisfy this requirement.
All employees who have not complete the required Mandated Report training must complete the on-line training and submit the certificate of completion to the Human Resource Office. Click on the link below to access the training module and select "Registration".
