FMLA - Family & Medical Leave Act

The Family and Medical Leave Act (FMLA) entitles eligible employees of covered employers to take job-protected leave for specified family and medical reasons. The following forms need to be completed and sent to the PVSD Business Office in order to verify eligibility and process a Family Medical Leave.
