Homeschool Affidavit & Required Submission Forms
By clicking the link below, you will access the Pleasant Valley School District Homeschool Affidavit. This affidavit is a sample for your convenience. You may use this or choose one of your own, as long as it includes the necessary, required information.
The affidavit must be completed, signed and notarized, and submitted to the Assistant Superintendent before your program should begin. You may also opt to submit an Unsworn Declaration in place of the affidavit. The Unsworn Declarations, for both elementary school age and secondary school age, are also available in the links below. Regardless of which you choose, it must be accompanied by a list of the educational objectives (your curriculum), evidence of medical services (physical exam), immunization records, and evidence of a dental exam. Medical, dental, and immunization forms are available in the links below.
If your child is exempt from immunizations, medical or dental exams, kindly submit a letter of exemption, a sample of which can be found in the links below.
